Ok, I was arrested for DWI, first time offender. I did provide a breath speciman and was over the legal limit. I spent 12 hours in jail, got out on a personal bond. When I got home and was going through my things that were in my car, I noticed that my TDL was missing. Upon looking up research on this (your web-site) I found out that I guess upon my release they were suppose to give me this DIC-25 Notice of Suspension form. I never got this form and I'm trying to figure out now, how to obtain it. I find your web-site very helpful and hope you can provide me with answers as to how to obtain this form.
